Beware of fraudulent phone calls

Someone claiming to represent Public Health has made several phone calls to people at home, asking them to take a community H1N1 vaccine survey. The caller asks for personal information such as date of birth, age, and zip code before abuptly hanging up. Please do not respond to any requests for personal information over the telephone or by e-mail.
