Helping Young Adults with Serious Mental Health Needs Transition into Adulthood (PDF - 309K) |
In September 2002, Clark County Department of Community Services & Corrections was awarded a four-year grant from the Substance Abuse & Mental Health Services Administration to fund the Partnerships for Youth Transition project in Clark County.
The goal of this project is to build a system of treatment to address the particular difficulties that youth with serious emotional disturbances or serious mental illnesses face in making a successful transition to adulthood. This comprehensive continuum of services will build upon existing programs and work to bridge gaps between the children's mental health system and the adult mental health system. Partners have agreed to participate in the planning, share data, cooperate with the evaluation of the program, and - once the process of designing services is complete - cooperate in serving youth.
The first year of this four-year grant was dedicated to planning. An oversight/planning committee consisting of 50% youth/family involvement meet monthly for the first year to establish the community plan for the Partnerships for Youth Transition project. The next phase of the grant, implementation, began in the grant's second year.
